Teacher Evaluation Nomination Form

Applicants for primary school must submit one evaluation from:

  • A classroom or homeroom teacher from the current or most-recent school.

Applicants for secondary school must submit evaluations from:

  • An English teacher from the current or most-recent school,
  • A Maths teacher from the current or most-recent school,
  • And a secondary school leader, such as a current school counselor, administrator, or other school leader who has regular interactions with the student.
