Student Health Insurance

Dear Parents

Please kindly be updated that the Vietnam Social Insurance Agency just issued Decision No.595/QD-BHXH providing guidelines on procedures of managing Health Insurance Card dated 14 April 2017, which took effect from 1 July 2017 (school has been applying this procedure for new students who started in September). Particularly, the social insurance department is changing the management method from managing through unit to individual management in order to create a central database for social insurance management by a unique  Social Insurance code (SI Code) for each individual in the whole country.


We are prepared to assist you all with this transition but need your help. By the 15th November please can you provide information to us as follows:

To renew the medical insurance card for BVIS students before 1st of Jan 2018, every student must submit the TK1-TS form with SI Code as the instruction below:

Parents are requested to fill in and sign the TK1-TS form which should include:  ward, district, province, the city has registered birth certificate. If the place of birth registration certificate cannot be identified, writing the birth-place (in cases of merger or splitting of administrative boundaries, writing the name at the time of declaration), registered resident place or temporary resident place.

How to look up student’s SI code?

Step 1: Select City/Province

Step 2:  Input full name of student

Step 3: Input D.O.B or Year of Birth of student

Step 4: Identify code provided

Step 5:  Look up

What will I do if I cannot look up SI number?

In case, parents cannot look up SI number, please provide detailed information on the appendix on members in family that attached together with the TK1-TS form. In which: providing full and accurate information of all household members in the household registration book or temporary residence book.

Who I submit the form to?

Submit TK1-TS form and appendix on members of family (if any)to Ms Hà- Cashier at Primary Office by 15th of November.

Note: The school will not be able to register medical insurance card for students if we do not receive this information by 15th November 2017. By not providing all information required by the social insurance body mentioned above by the 15th Nov deadline, parents have confirmed that they do not want to register for the medical insurance card for their children for 1st January 2018.
